Traumatic lower extremity injuries are challenging for reconstruction. Especially high-energy traumas may cause full-thickness and large tissue defects that cannot be closed with local flaps. Distal lower extremity injuries are inherently challenging injuries because bone, tendon and neurovascular structures are easily exposed. Therefore, reconstruction with a free flap is often the most logical option in such injuries.
